Service Items

1. English Proficiency Certificate

• Required Documents: ID card, English name (must match the name on the passport), and National Health Insurance card.
• Processing Time: Three Days
• Fees: 150 NT dollars for the first copy, 40 NT dollars for each additional copy.

2. English Birth Certificate

• Required Documents: The names of the applicant and their parents must be provided in English and should match those on the passport. Additionally, a birth certificate issued by this hospital in China is required.
• Processing Time: Three Days
• Fees: 150 NT dollars for the first copy, 40 NT dollars for each additional copy.

3. Medical Summary

• Required Documents: Personal National Health Insurance card, identification card, trustee identification card, and power of attorney.
• Processing Time: Three Days
• Fees: An administrative fee of 100 NT dollars applies. The cost for photocopying each page of a medical summary or test report is 2 NT dollars.

4. Medical Records

• Required Documents: Personal National Health Insurance card, identification card, trustee identification card, and power of attorney.
• Processing Time: Three Days
• Fees: An administrative fee of 200 NT dollars applies. Photocopying each page costs 2 NT dollars.
