Initial Consultation Registration

If you have never visited this hospital for a consultation, or if it has been more than seven years since your last visit, you must register as a new patient. If you have previously visited this hospital, you can register using your ID number. Please bring your ID and health insurance card to the service desk on-site to complete your medical history form.

After filling out the form, proceed to the registration counter in the outpatient lobby on the first floor (window 1-11) to take a number and continue with the registration process in order.

1. Online Registration

Registration Hours: Our service is available 24 hours a day, with a two-month advance booking period. Please note that same-day consultation slots cannot be reserved. If you are unable to register after three attempts, kindly contact us at 03-479-9595 ext. 326207 for assistance.

2. On-Site Registration

Registration Hours:

• Morning Outpatient: Starting at 6:00 AM, please take a number from the machine located in the outpatient lobby on the first floor. Registration begins at 7:50 AM and continues until capacity is reached. The registration cutoff time is 11:30 AM.
• Afternoon Outpatient: Starting at 6:00 AM, please take a number from the machine located in the outpatient lobby on the first floor. Registration begins at 7:50 AM and continues until capacity is reached. The registration cutoff time is 4:30 PM.
• Evening Outpatient Services (Monday, Wednesday, Friday): Registration begins at 7:50 AM and continues until capacity is reached. The registration cutoff time is 8:00 PM.

Registration Location:

The registration counter is located at window 1-11 in the outpatient lobby on the first floor.

Important Notes:

• Please bring your health insurance card and identification when you register.
• Registration is limited to same-day outpatient services, and each individual in line may take only one number if the number of consultations is limited.
• Since this is a centralized registration system for multiple departments, please specify the department, type of consultation, or the name of the doctor when registering on-site to assist the staff.
• On-site registration begins at 7:50 AM, and no advance reservations are accepted for same-day registration. For those wishing to register for outpatient services the following day, please utilize the online registration system, voice registration, or the touch-screen automatic registration machine.
• The registration fee will be collected along with the billing after the doctor’s consultation.

Health Check-Ups

Health checkups available to the public include services related to employment, education, examinations, long-term care, and support for foreign nationals, such as migrant workers and residents. The health checkup center is located on the left side of the first floor of the medical building.
