Introduction

1.Service Targets

The eligibility criteria for long-term care services require individuals to have at least Level 2 long-term care needs and to meet one of the following conditions:

  • Elderly individuals aged 65 and older.
  • Individuals who possess a manual disability certificate.
  • Indigenous Peoples aged 55 to 64.
  • Individuals aged 40 and older who have been diagnosed with dementia.

2.Comprehensive Care Model in the District

The development of the community-integrated care model at Level A (Community Integrated Service Center) is led by regional units that are equipped to deliver services. By promoting service integration and collaboration, local long-term care resources are strengthened.

The roles of service units at different levels are described as follows:

  • Level A Unit (Community Integrated Service Center): Responsible for developing care service plans for individuals with disabilities and for connecting them to, or providing, long-term care services.
  • Level B Unit (Complex Service Center): Specializes in providing long-term care services, which include:
    • Home services
    • Daycare
    • Family care
    • Home nursing
    • Community and home rehabilitation
    • Transportation services
    • Meal services
    • Assistive device services
    • Respite care

    These offerings are collectively referred to as the four major long-term care packages.

  • Level C Unit (Alley Long-term Care Station): Provides:
    • Social participation
    • Health promotion
    • Communal dining services
    • Programs aimed at preventing and delaying disabilities

3.Fee Standards and Subsidy Guidelines

The fee and subsidy standards are updated in accordance with the payment and reimbursement criteria established by the Health Bureau for long-term care. This includes:

  • Care services
  • Professional services
  • Transportation services
  • Assistive device services
  • Home accessibility improvement services
