Our hospital’s discharge preparation service team consists of skilled medical professionals, including physicians, nurses, social workers, nutritionists, respiratory therapists, rehabilitation therapists, and home care nurses. We are committed to providing patients with comprehensive discharge preparation services throughout their hospitalization. By promoting collaboration among the medical team, patients, and their families, we facilitate a smooth transition for patients returning home or being referred to appropriate chronic care facilities. Additionally, after discharge, we coordinate with community resources to ensure that patients receive ongoing, accessible, and comprehensive care.

  2. Provide information on diseases and guidance for care.

  5. Guide the application process and offer rental consultations for a variety of home care assistive devices.

  7. Provide consultation and facilitate the application process for Long-Term Care 2.0 resources.
